As a result, the immobilization system is not active during the learning procedure for a new "unlearned" blank replacement key could be used during a 30 minute reprogramming procedure to disable the immobilization system. Issue: On certain vans, the seat belt and airbag warning lights will not turn on for approximately 5 seconds after the ignition is switched to the ON or start position. Canadian regulations require these lights to turn on to demonstrate that the airbag system is working properly (bulb check) and to remind a driver to fasten their seat belt. Safety Risk: Warning lights that do not turn on during the start-up check could confuse a driver. In addition, the driver would not see a reminder to fasten their seat belt. This could create an increased risk of injury in a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a dealer to reprogram the instrument panel cluster.

Issue: Certain vehicles were built with the wrong type of glass in one or both of the rear-quarter windows. Tempered glass was used in place of laminated glass. As a result, the vehicles may not meet the ejection mitigation requirements of the Canadian regulations. Safety Risk: Vehicles built with incorrect side window glass could create an additional risk of injury in a rollover or side impact crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a dealer to replace the tempered window glass with laminated glass. For long wheelbase vehicles, the left rear-quarter window will be replaced. For short wheelbase vehicles, both the left and right rear-quarter windows will be replaced.

On certain vehicles equipped with Kidde fire extinguishers with black plastic handles and valves, the fire extinguishers may become clogged or require excessive force to activate. Also, the nozzle may detach from the valve assembly with enough force that it could cause injury, and fail to discharge. In case of fire, failure of the extinguisher to function could result in injury and/or property damage. Certain vehicles equipped with a hinged left side rear door may have a condition where inadequate clearance between the rear brake lines and the lower fuel filler hose clamp may result in abrasion to one or both rear brake lines. This condition could cause brake fluid loss which could result in decreased brake line pressure, increased brake pedal travel and increased stopping distance, increasing the risk of a crash causing injury and/or damage to property. This condition could cause various warning lights to illuminate in the instrument panel of the vehicle such the red “BRAKE” telltale symbol; illumination of the Stabilitrak telltale symbol; scrolling display of “Service Brake”, “Service Stabilitrak,” and “Service Traction Control” messages in the Driver Information Centre (DIC); and audible alerts/chimes. Correction: Dealers will inspect the brake lines. If the clearance is found to be below specification, the brake lines will be replaced with a new part design that provides adequate clearance.

On certain vehicles, underbody shut-off solenoid connectors for the compressed natural gas (CNG) fuel tanks may corrode and form a high-resistance short. This could result in a vehicle fire causing property damage and/or personal injury. Correction: Dealers will replace the solenoid at the fuel tanks and the regulator, and the 30 amp fuel pump fuse will be replaced with either a 7.5 amp fuse (for the four tank configuration) or a 5.0 amp fuse (for the three tank arrangement). In addition, the wiring routing will be adjusted, if necessary, to eliminate any undue tension on the connector, and anti-corrosion sealing plugs will be installed into the valve body.

On certain Cutaway vans, the transmission may self-engage into first gear while the vehicle is traveling at highway speeds, thereby causing transmission damage to certain gears. Should this occur at speeds beyond 130 km/h, the engine may shut down (and be able to be restarted with limited operating capability). Certain vehicles fail to comply with the requirements of CMVSS 108, requiring the illumination of the stop lamps whenever the brakes are applied. The rear brakes may still be applied although the brake pedal is released and the stop lamps are no longer illuminated. Certain vehicles may have been built with a power steering hose that is not to specification. Under extreme steering manoeuvres, such as turning the steering wheel fully to the left or right while braking, the hose may fracture and leak fluid. If this were to occur, power steering assist would be lost and increased steering effort would be required. On vehicles equipped with hydro-boost power brakes, it could also result in loss of power brake assist and increase braking effort would be required. If power steering fluid were to spray onto hot engine parts, an engine compartment fire could occur. On certain vehicles, the hydraulic brake booster's pressure accumulator may crack and/or separate from the Hydro-Boost assembly during normal vehicle operating conditions. If a separation occurred and the hood of the vehicle was open, fragments from the accumulator could cause injury to people in the immediate area. In addition, the presence of this crack or fractured surface could allow the hydraulic fluid to leak from the accumulator circuit of the booster assembly. The loss of fluid would cause increased steering and braking effort. Correction: Dealers will test and, if necessary, replace the Hydro-Boost assembly.

If this happens during braking applications, the driver may be able to hear an engine compartment noise similar to the sound that occurs when the steering wheel is turned to a full stop position. On certain vehicles, water may enter and corrode the rear licence plate lamp socket causing it to operate intermittently or not at all. The left rear tail lamp may also become inoperative. The temperature of the licence plate lamp socket could increase to the point that it may begin to soften and melt. If the socket assembly were to reach the point of melting, the occupants of the vehicle may notice an odor and smoke at the rear doors of the vehicle. On certain vehicles, the seat belt push button may not release the latch plate after the pretensioner is deployed, or alternately the seat belt latch plate may eject during a crash event with a pretensioner deployment. If an occupant was using the seat belt assembly and the latch plate ejected from the buckle assembly during pretensioner actuation, the seat belt assembly may not provide a desired level of occupant restraint. If an occupant was using the seat belt assembly and the push button became stuck in the buckle assembly during pretensioner actuation, pressing on the seat belt push button would not release the latch plate from the buckle to allow the belted occupant to exit the vehicle. Correction: Dealer will replace the front seat belt buckles.

On certain 2500 and 3500 series vehicles, a wheel bearing assembly retaining nut was omitted during assembly, which could lead to a possible bearing separation. If this were to occur, the ABS warning lamp would illuminate and the ABS system would be disabled. The driver would also notice a grinding noise, a decrease in vehicle speed and a steering pull in the direction of the affected assembly. Failure to heed these warnings can result in loss of vehicle control and a crash. Correction: Dealers will inspect and replace the wheel bearing assembly if a missing nut is noted during inspection.
